Hydroxyapatite CLS Hip Prosthesis Combined with Metal on Metal Articulation in Patients Younger than Forty-five Years Old

Abstract
Purpose: This study was undertaken in order to evaluate the minimum 5-year results of total hip arthroplasty, using hydroxyapatite(HA) coated CLS stem and rough-blasted(RB) Wagner cup combined with metal on metal articulation in patients younger than 45 years old.

Materials and Methods: From Jan. 1994 to Feb. 1996, thirty-eight total hip arthroplasty procedures were performed in patients younger than 45 years old using HA coated CLS stem combined with metal on metal articulation. The most frequent diagnosis was avascular necrosis of femoral head (82%). We evaluated both clinical and radiographic parameters.

Results: The mean Harris hip score was 97.2, improved from a preoperative 53.6. Radiographic findings demonstrated that endosteal bone formations around the stem were present in more than 90% of the cases in Gruen zone 1, 2, 6, 7 as well as in 40 to 70% of the cases in zone 3, 4, 5. In 7 cases (18.4%), the femoral metaphyseal medullary cavity was not filled completely by the femoral stem, although all exhibited excellent bone formation around the stem. Bone formation around the cup was present in 87, 95, and 79% in zones 1, 2, 3 respectively. No osteolysis around the stem and no measurable subsidence of more than 2 mm of the stem were seen. The mean linear wear rate was too low to be measured.

Conclusion: The low wear rate and excellent functional and radiographic results of arthroplasty using the HA coated CLS stem and RB Wagner cup combined with metal on metal articulation were observed, although we performed the procedure in active patients younger than 45 years old.
